Athletic Participation Forms

If your student is planning to participate in a sport this year, you MUST fill out the forms online at the link provided below.  We recommend registering online Before you get your physical so you can print the physical papers from Final Forms.

Follow these steps to ensure your athlete can participate:

Step 1:   Complete All Forms 

Step 2:   In FinalForms, print the “KSHSAA PPE Physical” and take it to your doctor for completion and signatures.

Step 3:   E-mail or hand deliver p.3 & 4 (only) of the physical to Jamie Jackson         (jamie.jackson@usd305.com) at South Middle School
