Athletic Participation Forms
If your student is planning to participate in a sport this year, you MUST fill out the forms online at the link provided below. We recommend registering online.
Follow these steps to ensure your athlete can participate:
Step 2: In Bound, print the PHYSICAL FORM and take it to your doctor for completion and signatures.
Step 3: Upload or hand deliver pg. 4 & 5 (only) of the physical to Stefanie Adam (stefanie.adam@usd305.com) at South Middle School
