As a friendly reminder, if your student is planning to participate in a sport during 2023-24 school year you must complete all district forms, including a sports physical before practice starts. Like the high school, we are using FinalForms software to complete all forms necessary to participate.
Follow these steps to ensure your athlete can participate:
Step 1: Register and complete ALL forms at: https://salina-ks.finalforms.com
Step 2: In FinalForms, print the “KSHSAA PPE Physical” and take it to your doctor for completion and signatures.
Step 3: E-mail or hand deliver the physical to Jamie Jackson ([email protected]) at South Middle school
